Default MOT Fail: headlamp pattern

My connoisseur failed its MOT. The test said the pattern of light from the offside lamp was "all over the place" his wording. I assume this means the bulb is not at the focal point of the reflector. The bulb has been installed correctly. So how does this happen It looks like I'll need a new headlamp unit. I am puzzled how this could have happened The plastic lens at the front of the lamp is clear.

Check the adjustment arms haven't popped out or that the bulb isn't in upside down, has been known

Check the bulb it could be faulty it does happen many millions made. Chris.S.
