Default 2002 ZT-T+ CDTI drive problem

I would be very grateful if anyone can advise on the following:
The car has done approx 150K and appears to be in top condition. The only problem is that there is a serious jerking (leapfrogging?) motion from standing start and at low revs in second. I followed the advice on this site and totally powerbled the clutch. The fluid was filthy but apart from the bite point improving the jerking still remains.

The service history with the car lists new clutch within last 18 months (cant define mileage since) new slave and master within similar timeframe. There is no record of the DMF being replaced during the life of the car.

I really dont want to pay top dollar for a DMF if thats what is required, so would be grateful if anyone can supply a part number for a replacement so that I can source one on ebay or suchlike. Any recommendations of good vendors (anywhere) also appreciated.......

Many thanks David,
Could it really be so simple? I've just got a Haynes so will check this mount before I go the expensive route. Wouldnt this give similar symptoms throughout all gears not just when pulling away?
