Glue the insert in with Superglue (probably after you've got the bolt out as previously suggested) then use the original bolt. The bolt might come out if you apply a sideways force to try to jam the insert.

I made some inserts over a year ago and I've not had a problem with the lens coming loose. Time consuming but a sense of achievement... ooh and cheaper then the kit on ebay
