Default Tensioner pulley

Hi. A garage has put a new tensioner on rover 75 diesel but keeps loosing the belt. I can move the tensioner very easy with a 24mm spanner and it doesn't return or have any pressure. Is it the back awkward part that should be replaced again or the arm. They had 2nuts holding arm into tensioner which I thought was unusual. Thanks for any replys. I have looked around the forum but unsure what I'm looking for 🤣
