Default KV6 Cambelt snag - Help?

I'm at a bit of a loss here. I've got the whole thing in bits and have had for a week now. The first, cheap, tool set didn't fit at all (I had to butcher the brackets to get them to reach the socket on the exhaust cam), so I've just got another rather more pricey one.
So, here's the adaptor:

Here's the tool fitted to the inlet cam on the front bank:

...and here's the resulting alignment at the rear:

..which looks to be about a tooth out to me.

So what on earth, if anything, am I doing wrong? Two toolsets now, one of which had to be "fettled" to fit and the resulting alignment is identical. Needless to say it is, if anything, worse with the 2 litre adaptor fitted.

Any ideas?
