The slipping is either due to wear on the clutch plate, it is seizing on the splines due to no grease being applied when the clutch was last replaced, weakening or broken diaphragm spring or contamination of the drive plate by oil leaking past the drive shaft seal.

Haven't come across the last two on our cars mind.
